Tecvayli-Talvey Combo Cuts Myeloma Progression Risk by 89%

Tecvayli plus Talvey cut the risk of disease progression or death by 89% in patients with relapsed multiple myeloma, according to new trial results.

The combination of Tecvayli (teclistamab-cqyv) plus Talvey (talquetamab-tgvs) reduced the risk of progression or death by 89% compared with standard treatment among patients with relapsed/refractory multiple myeloma, according to results from the phase 3 MonumenTAL-6 trial.

"These findings add to a growing body of phase 3 evidence evaluating the survival outcomes associated with the early use of immunotherapy doublets in the treatment journey," Dr. Ajay K. Nooka, director of the myeloma program in the Department of Hematology and Medical Oncology at Emory University School of Medicine, said in a press release issued by Johnson & Johnson.

"Tecvayli and Talvey together generated deep and durable responses, demonstrating what's possible by targeting BCMA and GPRC5D at the same time, and further reinforcing the potential of this off-the-shelf regimen to improve outcomes for patients across practice settings."

In addition, the combination reduced the risk for death by 62%, which "represents the lowest hazard ratio seen across any Phase 3 study evaluating bispecific therapies in relapsed/refractory multiple myeloma," according to the pharmaceutical company's release.

What did the phase 3 MonumenTAL-6 trial evaluate?

Researchers looked at a new way to treat multiple myeloma that has returned or stopped responding to previous treatment. They tested two drug combinations versus the investigators' choice of standard of care:

• Tecvayli plus Talvey
• Talvey plus the immunomodulatory drug Pomalyst (pomalidomide)

The trial enrolled adults whose multiple myeloma had returned or stopped responding after one to four earlier treatments, including two of the most common early therapies. Patients were then randomized — assigned by chance to one treatment group or another.

What did topline results from the phase 3 MonumenTAL-6 trial show?

Patients who received Tecvayli plus Talvey were 89% less likely to have their cancer worsen or die during the study, compared with patients who received standard treatment. Similarly, patients who received Talvey plus Pomalyst were 73% less likely to have their cancer get worse or die.

As a result, the trial met its primary end point of progression-free survival.

Full results will be presented at a future medical conference and shared to global health authorities.

What are the side effects associated with Tecvayli and Talvey?

Researchers reported that the safety results for both combinations tested in the study were consistent with what's already known about each drug on its own.

Tecvayli and Talvey both carry a boxed warning for two risks: cytokine release syndrome and neurologic toxicity. The pharmaceutical company noted that both drugs are only available through a program called the Tecvayli and Talvey Risk Evaluation and Mitigation Strategy, so any reaction can be caught and treated quickly.
