Interleukin-2 (IL-2) is often the most challenging part of tumor-infiltrating lymphocyte (TIL) therapy, and Michael Goldman describes firsthand how the rigors began about 30 minutes after each dose, building into 20 to 30 minutes of intense shaking and fever before resolving. He recalls feeling fine through his first 4 doses of IL-2 and then unexpectedly ending up in the intensive care unit after the fifth. Medical oncologist James Smithy, MD, MHS, of Memorial Sloan Kettering Cancer Center offers the clinical perspective on what likely happened. He explains that a probable seizure was triggered by inflammation near brain metastases, which placed Michael at higher risk for this complication. Both reflect on how quickly the ICU team stabilized him within roughly 24 hours, and how, despite the scare, his brain disease has remained well controlled since treatment.
